Creating Healthy Places to Live, Work & Play in Herkimer County

Creating Healthy Places to Live, Work and Play


In order to combat obesity, diabetes and promote physical activity and improved nutrition in Herkimer County through environmental and systems change; Herkimer County HealthNet, Inc. (HCHN) has been awarded a “Creating Healthy Places to Live, Work and Play” grant by the New York State Department of Health. The major focus areas of “Creating Healthy Places” activities are:


• Education of policy and decision makers to increase awareness of and build support for obesity and type 2 diabetes prevention issues, and increase knowledge about and action to implement evidence-based prevention strategies,
• Advocacy with community and organizational decision makers to promote adoption and implementation of policies, systems and environments that promote and sustain healthful eating and physical activity,
• Education of community members to build support for obesity and type 2 diabetes prevention efforts and stimulate demand for policy, systems and environmental changes to improve healthful nutrition and physical activity opportunities,
• Utilization of earned media to advance obesity and type 2 diabetes prevention action to keep the obesity and diabetes epidemics on the public agenda and frame the issues for community members and decision makers.


HCHN will concentrate in the following areas:


Environmental approaches to increase availability of places to be physically active;

• Establish/improve and promote use of neighborhood or community trails,
• Create, rehabilitate, maintain and promote parks and recreation facilities in underserved residential areas to offer safe, accessible and attractive opportunities for physical activity for persons of all ages and abilities,
• Implement the Nutrition and Physical Activity Self-Assessment for Child Care program (NAP SACC) television viewing/screen-time reduction module, developed by NYSDOH, in 1-2 new childcare and daycare settings;


Environmental approaches to create community landscapes conducive to physical activity;

• Improve land use policies, building codes, roadway design standards, zoning regulation and other governmental changes that promote and facilitate physical activity. For example implementing Complete Streets Legislation.


Increase availability of fresh fruits and vegetables & healthful quality of foods offered for sale;

• Establish and promote the use of farmers markets and implement innovative strategies to increase access to healthy foods in high need areas,
• Implement NAP SACC in 1-2 childcare and/or daycare settings
• Enhance variety of fruits and vegetables in grocery stores, convenience stores and small grocery stores by enhancing the visibility and increasing the overall shelf space allocated to healthful foods.

About HealthNet

The HCHN has been in existence since 1990. Its membership has continuously expanded and has served as an effective forum for Herkimer County providers to identify, discuss, and resolve health and social service issues and concerns. As a direct result of grants by the New York State Department of Health: Office of Rural Health, the Network significantly increased levels of coordination and collaboration among health and social service providers.
